The Center's highly skilled physical therapists will provide experienced, compassionate physical therapy to patients along with educating families and caregivers, all in the home.

The Center uses a third party* to track outcomes, proving we have a lower number of visits per diagnosis than our State of Michigan peers, helping providers/patients reduce their out-of-pockets costs and accelerating the transition from home to outpatient care.

Your home health therapist will have direct communication with your outpatient therapist. This will ensure that your personalized treatment plan and goals are conveyed effectively, ensuring a smooth transition from home health to the outpatient setting.
